ABSTRACT Objective Adhesive capsulitis is an inflammatory disease of the joint capsule, clinically manifested as pain, stiffness, and dysfunction of the shoulder. We subjectively observed an increased incidence of adhesive capsulitis, and raised the hypothesis that adhesive capsulitis was more frequent in magnetic resonance imaging examinations performed during the COVID-19 pandemic as compared with examinations prior to this period. Methods Data from medical records and magnetic resonance imaging of the shoulder presenting typical imaging findings of adhesive capsulitis, performed in our organization from March to June 2020, were evaluated and compared with data and imaging from the same period of the previous year. To this end, an organizational business intelligence tool called “search reports” was used, searching for the term “adhesive capsulitis” in the radiological report, results were tabulated, and corresponding magnetic resonance imaging exams were analyzed. Results Our search found a total of 240 and 1,373 cases of adhesive capsulitis in the 2020 and 2019 periods, respectively. The mean age of patients was 53.9 years in the 2020 group and 49.9 years in 2019 (p<0.001). Magnetic resonance imaging findings were positive for adhesive capsulitis in 40 out of 240 shoulders (16.7%) in the 2020 group versus 127 out of 1,373 shoulders (9.2%) in the 2019 group. This difference was statistically significant (p=0.001). Conclusion Our study findings suggest a relative increase in the proportion of magnetic resonance imaging findings suggestive of adhesive capsulitis cases during COVID-19 pandemics based on data from our organization.

ABSTRACT The objective of this article is to describe births according to their distribution by day of the week in order to characterize the scheduled or non-scheduled aspect of the labor and delivery process. A descriptive epidemiological study of the temporal distribution of the days of the week on which births occurred in the Autonomous City of Buenos Aires was carried out for the period 2004-2013, based on the Statistical Reports of Live Births database of the Directorate of Statistics and Health Information at the National Ministry of Health. The following variables were analyzed using descriptive statistics: establishment (public or private sector), gestational age, maternal age, and mother’s level of education. The analysis reveals that births decrease significantly on Saturdays and Sundays in both public and private establishments, with a greater decrease being observed in private establishments. Births at week 37 are more frequent in private establishments and decrease on non-working days. For mothers with higher levels education, fewer births occurred on weekends. Analyzing the day of the week on which births occur reveals the medicalization of the delivery process and could indicate that the scheduling and induction of births occur in private establishments to a greater extent than in public establishments.

RESUMEN El objetivo de este trabajo es describir los nacimientos según su distribución en días de la semana a fin de caracterizar el aspecto programado o no del proceso de parto y nacimiento. Se realizó un estudio epidemiológico descriptivo de la distribución temporal de los días de la semana en que ocurrieron los nacimientos en la Ciudad Autónoma de Buenos Aires, para el período 2004-2013, a partir de la base de datos del Informe Estadístico de Nacido Vivo de la Dirección de Estadísticas e Información de Salud del Ministerio de Salud de la Nación. Se analizaron las siguientes variables: establecimiento (público o privado), edad gestacional, edad y nivel de instrucción de la madre. El análisis muestra que los nacimientos disminuyen significativamente los sábados y domingos en los establecimientos públicos y privados, siendo mayor la disminución en los establecimientos privados; que los nacimientos en la semana 37 son más frecuentes en los establecimientos privados y disminuyen en los días no laborables; y que a mayor nivel de instrucción materna son menores los nacimientos durante los fines de semana. Analizar el día de la semana en el que ocurren los nacimientos transparenta la medicalización del proceso de parto e indicaría que la programación e inducción de los nacimientos ocurre en los establecimientos privados en mayor medida que en los establecimientos públicos.

ABSTRACT Just days after the Argentine Chamber of Deputies approved the bill on the Voluntary Termination of Pregnancy, Salud Colectiva asked Dora Barrancos and Patricia Rosemberg to converse with representative Mónica Macha regarding the events leading up to the Chamber’s approval of the bill. In the conversation other themes emerged in connection to the energy that flooded the streets all over the country throughout the year. The three women highlighted the activism and sorority present inside and outside of the chamber, the central role of youth, “the revolution of the daughters,” the discussion of body autonomy, the role of doctors and how the struggle became a cultural change from which there is no return. This conversation narrates a historical moment in the words of some of its actors.

En la charla surgieron otras temáticas, que inundaron las calles de todo el país durante todo el año. Las tres recuperan la militancia y la sororidad dentro y fuera del recinto, el protagonismo de la juventud, “la revolución de las hijas”, la discusión sobre la autonomía de los cuerpos, el rol de los médicos y cómo todo esto devino en un cambio cultural del que ya no hay vuelta atrás. Objective Investigate the differences in cardiopulmonary (CP) capacity and Quality of Life (QOL) between healthy elderly (≥ 65 years) with different TSH levels (< 1.0 and ≥ 1.0 μIU/mL) both within the normal range. Also, evaluate the effects of TSH elevation on CP test and QOL, by administering methimazole to subjects with initial lower-normal TSH, in order to elevate it to superior-normal limit. Materials and methods Initially, a cross-sectional study was performed to compare CP capacity at peak exercise and QOL (using WHOQOL-OLD questionnaire) between healthy seniors (age ≥ 65 years) with TSH < 1.0 μIU/mL vs. TSH ≥1.0 μIU/mL. In the second phase, participants with TSH < 1.0 μIU/mL were included in a non-controlled-prospective-interventional study to investigate the effect of TSH elevation, using methimazole, on QOL and CP capacity at peak exercise. Results From 89 elderly evaluated, 75 had TSH ≥ 1 μIU/mL and 14 TSH < 1 μIU/mL. The two groups had similar basal clinical characteristics. No difference in WHOQOL-OLD scores was observed between groups and they did not differ in terms of CP function at peak exercise. QOL and CP variables were not correlated with TSH levels. Twelve of 14 participants with TSH < 1.0 μIU/mL entered in the prospective study. After one year, no significant differences in clinical caracteristics, QOL, and CP variables were detected in paired analysis before and after methimazole intervention. Conclusions We found no differences in CP capacity and QOL between health elderly with different TSH levels within normal range and no impact after one year of methimazole treatment. More prospective-controlled-randomized studies are necessary to confirm or not the possible harm effect in normal low TSH.

Pacientes diagnosticados com mieloma múltiplo são mais suscetíveis a infecções, que é a principal causa de morbidade e mortalidade associadas a essa doença. Os principais agentes infecciosos envolvidos são as bactérias Gram-positivas, mas, após a quimioterapia, pode haver aumento na incidência de Gram-negativos, que são responsáveis, na maioria dos casos, por infecções do trato urinário. Assim, descrevese um raro caso de um paciente de 73 anos de idade, com mieloma múltiplo diagnosticado com endocardite por pseudomonas.

Patients diagnosed with multiple myeloma are more susceptible to infections which are the major causes of morbidity and mortality associated to this disease. The main infectious agents involved are Gram-positive bacteria. However, after chemotherapy an increase in the incidence of Gram-negative strains is observed. These bacteria are also responsible for most cases of urinary tract infections. Here is reported a rare case in a 73-year-old man with multiple myeloma who developed endocarditis due to pseudomonas.
